Teeth in one day: myth or reality?

Today is a very exciting time in implant dentistry. While many older dental practices in NYC still use old style dental procedures that take over a year, Tribeca Advanced Dentistry NYC can do it in a single appointment. Thanks to significant advancements in dental technology, we are now able to restore an entire arch of damaged or missing teeth with dental implants in just one day. Teeth in a day dental procedure used to take 6 months to a year and required wearing a removable denture for the duration of the process. However, now you can have damaged teeth removed, implants placed, and teeth attached to the implants, all in a single appointment. What is even more exciting is that you often need only 4 implants to replace a full set of teeth on the top or the bottom. Placing fewer implants is more cost efficient, less intrusive, and does not require painful and expensive bone grafting. This dental procedure is also recommended for patients who are uncomfortable wearing denture and would like to have teeth attached to dental implants.

How does it work?

Teeth in a day procedure was developed by Nobel Biocare, a widely recognized leader in dental implants. Nobel Biocare invests a lot of resources into research and development of their products, making them superior to other implant companies on the market. This procedure enables clinicians to attach teeth to the implant at the same time with implant placement. Teeth in a day or immediate implant loading procedure utilizing 4 implants is also known as “All-on-4”. All-on-4 protocol uses so-called “tilted implants.” The four implant posts are strategically placed at a 45-degree angle for two reasons: first, to increase their strength and stability; and second, to avoid damaging the sinus cavity in the upper jaw or the nerve canal in the lower jaw. Tilting the implants enables them to anchor to the better quality jawbone, improving their support. Also, All-on-4 uses longer dental implants, increasing the contact between the implant and bone and reducing the need for bone grafting (in which the jawbone is supplemented to provide a better base for the implants). The implant bridge, which replaces a full set of teeth, is attached to implants with a screw so that patient can function comfortably with them right after the procedure is completed. These teeth look and feel very similar to natural teeth.

Is it painful?

Surprisingly enough, the procedure is much less painful that the conventional implant process that requires patients to wear a removable denture after implant placement. This is due to the fact that the new teeth are attached to implants and do not irritate the gums the way the removable dentures do. Also, there is no need for bone grafting which can be very painful. This procedure is usually done under sedation so the patient experiences little discomfort.

Prestigious “Inside Dentistry” magazine published an article by Dr. Malkin describing her case studies utilizing with all-on-4 dental implant procedure. This dental implant procedure is relatively new, only developed in mid-90’s and widely accepted today as a more comfortable and cost effective to the traditional implant placement, which often required patients to wear dentures as long as one year while waiting for implants to heal. Even today, many patients end up wearing dentures or undergo expensive and painful bone grafting to become a suitable candidate for implants if they have bone loss.

What is a cosmetic dentist?

In our New York dental office in Tribeca, we are sometimes asked if we have a cosmetic dentist or we specialize in cosmetic dentistry. This is a difficult and somewhat misleading question to answer correctly. Most people go see a dentist because they want to maintain or improve their appearance. Our practice specializes in treating advanced dental aesthetic and restorative cases.

Any dental work from simple such as teeth whitening to sophisticate such as dental implant placement can be thought as “cosmetic dentistry”. American Dental Association, the oldest and most prestigious association for American dentists, doesn’t recognize “cosmetic dentist” as a dental specialty. There is no residency for “cosmetic dentistry” post-dental school where a new graduate can receive specific training unlike those available for such specialties such as periodontists, prosthodontists, endodontists, orthodontists, and oral surgeons. We think periodontists, prosthodontists, and orthodontists come closest to the definition often ascribed to cosmetic dentistry. All these three specialties require three years of additional training. Periodontists such as Dr. Moldovan, are experts in treating gum (periodontal) disease, prosthodontists, such as Dr. Malkin, are experts in the restoration and replacement of teeth, and orthodontists treat improper bites and crooked teeth. Our Tribeca Advanced Dentistry New York office have both periodontist and prosthodontists who often work as a team to treat complex cases such as an all-on-4 procedure to replace diseased and broken teeth and immediately load dental implants. We believe it’s important that patients, especially in complex and difficult dental cases, are treated by dental experts with advanced post-graduate training.

Dental costs in New York

When it comes to dental costs in New York, it may seem that going to a good dentist is expensive. There are several factors to consider in determining true dental costs. While routine dental procedures such as dental cleaning and or fillings should cost similar amounts in New York, often dictated by the dental insurance, you may see that advanced aesthetic dentistry, such as all-on-4 teeth replacement, dental implants, or getting bridges and crowns can vary greatly in price. There are several components to the cost of an advanced dental procedure: dentist’s training, dental lab quality, and dental equipment.

For example, all our doctors have completed multi-year post-dental school training to obtain their specializations. Our prosthodontist, Dr. Malkin spent 3 years in her specialty in West Los Angeles VA Hospital, our periodontist, Dr. Moldovan continued her studies in UCLA’s post-graduate periodontics department, and our endodontist, Dr. Fleischer was a chief resident at Mount Sinai Hospital in New York.

For our equipment, in Tribeca Advanced Dentistry New York, we use a 3 dental imaging scanner for dental treatment planning by a prosthodontist, a microscope for endodontist, and lasers for periodontal treatment. While this modern equipment adds to the cost of our procedures, we believe it’s essential to the dental treatment success.

Finally, we have a long-term partnership with Beverly Hills Dental Lab located in Beverly Hills, CA for most of our critical dental lab work, which we believe to be one of the best in the country. We use almost exclusively Nobel Biocare dental implants and other critical parts, an acknowledged leader with over 30 years of clinical studies supporting its dental products. Their work and products are in general more expensive but we feel it’s absolutely worth the money as good dental work should last.

Yet, we also believe that the prices should be affordable enough for the middle class and accept most of the good PPO insurances fee schedules. We would rather have a practice with many happy patients who feel they received excellent dental care than try to sell the most expensive dental treatment.

How long will my dental implants last?

Here at Tribeca Advanced Dentistry’s downtown New York office, we get asked, “How long do dental implants last” quite often. It’s a great question, especially considering the fact that complex dental implants and full-mouth rehabilitation can be rather costly. Patients want to know that their All-on-4 Implant procedure is gong to last them for the long haul.

Many of our patients perceive dental procedures as long-term, but ultimately temporary – something that they will eventually have to spend the same amount of money on (if not more) in the future.

Let’s look at this from a different perspective. Dental implants have been around for thousands of years. In fact, anthropologists discovered crude versions of implants in ancient Egyptian mummies. However, it wasn’t until around 50 years ago that modern implant dentistry was born in Sweden, with the first modern dental implants being placed in 1965 by Dr. Per-Ingvar Branemark.

40 years after that original dental implant – the first ever modern dental implant procedure, mind you – the original implants were still in place when the patient died in 2005.

Since that time – with each new decade – researchers have made great strides in the quality of materials they use and the techniques they employ. Success rates have only increased.

In the present time, implants are made out of titanium that is chemically treated to improve compatibility with actual living tissue. Often, these implants are designed with 3D software for better placement and superior fit with other teeth.

Even though dental implants are typically more expensive than a dental bridge or dentures, they last much, much longer. Also, patients don’t suffer from bone loss with implants like they do with dental bridges that are done without implanted posts.

In fact, according to current research in implant dentistry, dental implants have a 95% survival rate after ten years, which is a figure estimated using multiple clinical trials and long-term follow-up studies. Basically, failure of a high-quality dental implant is relatively rare and, if done properly by trained oral surgeons, periodontists, or prosthodontists, should last a lifetime.

Not only that, most high-quality implants also have a lifetime warranty for both dental implants and implant parts. Typically, if a dental implant is going to fail, it occurs within the first six months where it usually fails to heal (osseointegrate). In that situation, the implant can easily be removed and replaced by another in a small amount of time.

When it really comes down to it, the best way you can ensure your dental implants last as long as they can is to maintain proper dental hygiene, schedule dental cleanings and checkups twice a year, and follow doctor recommendations. As dentists, we are unable to provide guarantees because an individual’s overall health is a factor beyond our control – factors such as nutrition, genetics, or disease.

So, although we here at Tribeca Advanced Dentistry can’t say 100% that your dental implants will last the rest of your life, we can say that healthy living, choosing a high-quality dental specialist for the procedure, and regular follow-ups should help you “tip the scales” in your favor, so to speak.
