All-on-4 dental implants replace a full arch of missing or failing teeth with a fixed bridge supported by four implants. For suitable patients, a temporary bridge can be attached on the day of surgery, providing fixed replacement teeth while the implants heal.

All-on-4 is a full-arch implant treatment concept developed through the work of Dr. Paulo Maló and Nobel Biocare. Four dental implants support a bridge that replaces the upper teeth, lower teeth, or both arches.
Unlike a conventional removable denture, the bridge is secured to implants. You do not take it out for daily cleaning, although your dental team can remove it when clinically necessary.
The teeth provided on the day of surgery are generally a temporary restoration. Your final bridge is made after the implants and surrounding tissues have healed sufficiently.
The traditional All-on-4 arrangement uses two generally straight implants toward the front of the jaw and two angled implants farther back. The rear implants may be tilted up to 45 degrees, depending on the treatment plan.
This arrangement makes use of available bone and can help avoid anatomical structures such as the upper-jaw sinuses. In selected patients, it can reduce or eliminate the need for bone grafting. The position and angle of each implant are determined through examination and imaging.

All-on-4 may be appropriate for people who have lost most or all teeth in an arch, wear removable dentures, or have extensively damaged teeth that cannot be predictably restored.
Dr. Malkin evaluates your remaining teeth, gums, bone support, medical history, and treatment goals before recommending a plan. Teeth that can be maintained should also be considered when discussing your options.
Having bone loss does not automatically rule out implant treatment. However, All-on-4 is not appropriate for everyone, and some patients need grafting, additional implants, or a different approach.
Suitable patients may receive a fixed temporary bridge immediately after implant placement. This depends on the treatment plan and whether the implants achieve sufficient stability during surgery.
The temporary bridge allows you to have fixed teeth during healing, but it does not mean treatment is complete or that you can immediately resume unrestricted chewing. You will receive instructions about a soft-food diet, cleaning, and protecting the implants.
If immediate loading is not appropriate, Dr. Malkin will discuss an alternative healing and temporary-tooth plan.

Teeth-in-a-Day describes the timing: replacement teeth are attached on the day of implant surgery in suitable cases.
All-on-4 can be one way to provide Teeth-in-a-Day. Other full-arch treatments may use more than four implants while still allowing an immediate temporary restoration.
All-on-4 offers a fixed alternative to removable dentures and can restore the appearance and function of a full arch of teeth without replacing every missing tooth with an individual implant.
For suitable patients, the approach can reduce the need for grafting and allow fixed temporary teeth during healing. The bridge is planned around your bite, smile, and facial appearance.
The benefits, treatment time, and overall cost depend on your individual needs. Follow-up visits and ongoing maintenance remain part of treatment.
Published studies have reported high implant survival rates with All-on-4 treatment. For example, a 2012 clinical study involving 36 patients and 44 full-arch restorations reported three-year implant survival rates of 98.96% in the upper jaw and 97.5% in the lower jaw. All 44 bridges remained in place during the study’s observation period.
These are results from a particular study, rather than a guarantee of an individual patient’s outcome or a success rate specific to our practice. Dr. Malkin will discuss the expected benefits, limitations, and risks of your proposed treatment.
Your treatment begins with an examination, a review of your medical and dental history, and appropriate imaging. Dr. Malkin evaluates your bone, bite, and remaining teeth and discusses your expectations for your new smile.

On the day of surgery, teeth that cannot be saved are removed as planned, and the implants are placed. When clinical conditions allow, a temporary bridge is attached that day.
Anesthesia and sedation options are discussed in advance based on your medical history and the planned procedure.
Healing takes several months. During this period, Dr. Malkin checks your implants, gums, and bite and makes adjustments when needed.
Once healing is sufficient—often around four to six months, depending on the case—the final bridge is fabricated and fitted. Materials may include zirconia or other restorative options selected for your clinical needs.
Your experience with the temporary teeth helps guide refinements to the final bridge’s fit, function, and appearance.

Treatment typically includes two main phases: surgery with the temporary restoration, followed by the final bridge after healing.
At your consultation, our team will explain the proposed treatment and associated fees. Your estimate should clearly identify what is included, such as extractions, implants, anesthesia, temporary teeth, the final bridge, and follow-up care.
When comparing estimates, check that they cover the same stages and services. A quote for surgery and temporary teeth alone is different from an estimate that includes the final restoration.
